The Health Office and the Nurse for St. Augustine School are served through the
Ossining School District
and as such our students must be compliant with the health guidelines as noted below.
As we prepare for the 2025 – 2026 school year,
please be aware of the following health requirements
New York State requires physical exams for all new entrants,
and students entering Grades Pre-K or K, 1, 3, 5, & 7 in September 2025.
Physical exams must be done on the mandated NYS Health Exam Form or the required EHR equivalent. The form may be accessed by clicking here for a fillable pdf. Please have your doctor fill it out sign and then return it to the school office either by scanning and emailing or bringing it in to Nurse Katie Sansone.
In order to attend or remain in school, children who are unvaccinated or overdue must receive at least the first dose of all required vaccines within the first 14 days. They also must receive subsequent vaccines in the series within a 14-day period of when they are due to complete the immunization series. Students who do not have acceptable evidence of immunization or immunity and are not considered in-process or who do not have an acceptable medical exemption will not be permitted to attend school,
Immunization Medical Exemptions
A student may continue to attend school without the required immunizations if they have a valid medical exemption. A medical exemption is a written statement/form from New York State Department of Health completed and signed by a physician licensed to practice medicine in NYS certifying that the immunization may be detrimental to the child’s health. It must contain sufficient information to identify a medical contraindication or precaution to a specific immunization, and specify the length of time the immunization is medically contraindicated. Requests for medical exemptions must be completed annually. The district physician, superintendent, principal or designee may require additional information supporting the exemption request. Medical exemptions must be approved by the District Physician, Superintendent, Principal, or designee.
